Please read the rules before completing the registration form.
Forms are also available at Soccer Plus International,
Sport Chek, National Sports, Play It Again Sports and Dairy Queen
Registration Fee
is $160.00 (2002-2007)
(games played on 1/3 field)
Registration Fee
is $199.00 (1987-2001)
(games played on full field)
Season starts October 1st and
2nd 2011

Make cheque payable to "PYISC". Be sure to complete all sections and to
sign and date the form (both sides).
Players 18 & Over MUST sign Registration Form
Registrations received after August 1, 2011 will be subject to
a $10.00 late fee.
Registrations received after August 12, 2011
are not guaranteed team placement.
Refunds and NSF cheques are subject
to a $20 administration fee.
Registration forms will not be accepted
after December 31st, 2011.
